2013 MANUEL MAIR NPC SOUTHERN STATES MEN’S FITNESS & PHYSIQUE (OPEN, TEEN, MASTERS & NOVICE) CHAMPIONSHIPS OFFICIAL ENTRY FORM

SANCTION : The contest is sanctioned by the National Physique Committee of the USA Inc. (NPC). Sanction #12028.

ELIGIBILITY : This contest is open to all males in the U.S. Contestants must be a member of the NPC (fee for 2013 is $100.00) and membership cards will be available at registration.

HOTEL ACCOMMODATIONS: The official host hotel is the GALLERY ONE - FT LAUDERDALE DOUBLETREE SUITES HOTEL, 2670 E. Sunrise Blvd., Ft. Lauderdale, FL 33304. The hotel is located near the contest site. Every room is a suite with a kitchen (fridge, microwave, etc.), dining area and living room. A special discounted rate has been arranged. For reservations, call 1-954-565-3800- be sure to mention that you are part of the “SOUTHERN STATES CHAMPIONSHIP in order to get the discounted rate. MANDATORY CHECK IN: Thursday, July 11, 2013 – 5:00pm – 8:00pm –Gallery One Doubletree Suites Hotel, 2670 E. Sunrise Blvd, Ft. Lauderdale, FL 33304. Room to be designated.

In case of emergency or hardship, call the promoter for a check- in on Friday, July 12, 2013 – 8:00am – 9:00am – War Memorial Auditorium. Mandatory athlete meeting with Manuel Mair at 9:30am. PREJUDGING: Starts at 10:00am. Evening Show: 6:00pm.

AWARDS: 5 Places – 1Class plus Special Awards of BEST PHYSIQUE, BEST IN STRENGTH ROUND & OBSTACLE COURSE . All contestants will receive a contestant medallion and t-shirt. The 1st place winner will receive travel expense reimbursement of $250.00.

GENERAL INFORMATION: In Judging MEN'S FITNESS competition, the judges shall assess the athleticism of the physique. This is NOT a bodybuilding contest - huge muscle mass and deep muscle separations/striations will not be rewarded. The muscle tone should appear firm, full bodied and tight. The MEN'S FITNESS competitions provide an alternative for athletic men who train hard but do not wish to gain massive muscle size or the "cuts" required for bodybuilding contests.

ROUND 1/PHYSIQUE: Contestants wear either runners or biker shorts ( no posing suits) along with athletic shoes and socks. All contestants will be brought out individually for four quarter turns at center stage and then repeating the quarter turns in a line-up with the other contestants. The judges will be assessing overall symmetry, proportion, firmness and physical appearance including complexion, skin tone, poise and presentation that best shows an athletic physique.
ROUND 2/STRENGTH: The Strength Round consists of two exercises - Chin-Ups and Push-Ups. Contestants will be brought out individually in numerical order. While a fitness outfit is recommended, the costume/apparel will not be part of the judges' decision. Shoes are not required for this round. A time limit of one (1) minute for each event per individual has been set. The contestants will be judged on how many correctly executed Chin-Ups and Push-Ups accomplished in the 1 minute time period. Effort must be continuous with no resting. CHIN-UPS : No straps, gloves or chalk will be permitted. Chin-Ups may be done either with an underhand or overhand grip. A correctly executed Chin-Up consists of full extension of the arms on the bottom to the chin above the bar at the top done without stopping at top or bottom. PUSH-UPS : A line of tape will be placed on the stage and each contestant's toes shall remain behind the line. Contestants will face the audience. A correctly executed Push-Up consists of full extension of the arms at the top and down to touching the chest to the stage. After the rankings are determined for both exercises, each contestant's scores will be added together for one placement for this round. If there is a tie, the max pull-ups and if necessary, max push-ups will determine the tiebreaker.
ROUND 3/OBSTACLE COURSE: Timed event - short in duration. The obstacles will challenge your speed, stamina and agility. This event will be performed two at a time. The Obstacle Course will consist of 20 vertical jumps onto a 16" high bench (up down 20 times), 20 full range sit-ups, 30 jumps with a 3 lb. jump rope (must be complete jumps starting with rope behind your feet), 10 full range push-ups, 10 tires staggered drill and a 5 point shuttle (suicide) run with blocks to pick up to ensure athletes are touching the line. Athletes will receive a ranking based on the time to complete the course.
SCORING & AWARDS: Each of the 3 Rounds will be valued equally for 33 1/3% of the overall score. The final score/placement will represent all three rounds. If there is a tie, the athlete ranked highest in Round 3 will be determined the winner. All contestants will be brought back on stage at the night show for Round 1 and the top 5 in Round 2 & 3 will participate in a new scoring for each of these rounds. Medallions will be presented to every contestant. The Top five (5) finalists and winners of the Special Awards (, Best Physique, and Best in Strength Round & Obstacle Course ) will remain on stage for the presentation of Awards.

AWARDS: OPEN: 4 HEIGHT CLASSES-5 PLACES + OVERALL, TEEN MEN- 3 PLACES – 1 CLASS, MASTERS MEN (OVER 40 & 50) – 5 PLACES – 1 CLASS. NOVICE MEN - 1 CLASS - 5 PLACES. OPEN OVERALL WINNER will receive $250 travel expense reimbursement for the 2013 NPC USA on July 26-27 in Las Vegas, NV or the 2013 NPC Nationals on Nov. 22-23 in Ft. Lauderdale, FL.

MUSCULARITY & BODY CONDITION: Judges will be looking for fit contestants who display proper shape and symmetry combined with muscularity and over condition. This is not a bodybuilding contest so extreme muscularity will be marked down.
STAGE PRESENCE & PERSONALITY: Contestants will appear in board shorts without shirt and barefoot. The shorts must be just above the knee in length and can be 1inch below the belly button. No spandex or logos permitted except for manufacturer’s logo such as Nike. Judges are looking for the contestant with the best stage presence and poise who can successfully convey his personality.

PRESENTATION: Competitors will be called out in numerical order, walk to center stage and perform 4 quarter turns with optional pose of hand on hip or in pocket.
COMPARISON ROUND: All of the competitors in their height class will then be brought back out and do their 4 quarter turns as a group. Judges will have the opportunity to compare the competitors. At the night show, the winners of each of the 4 classes will be brought out for comparison & scoring for the overall.

NATIONAL QUALIFIER: MEN’S OPEN: The top 5 in each height class qualifies for the 2014 NPC Jr. USA & Jr. Nationals. The top 3 qualify for the NPC USA & Nationals for 2013, 2014 & 2015. TEEN & MASTERS – The top 5 qualify for the 2013 Teen, Masters & Collegiate Nationals.

CROSSOVERS: Contestants can crossover between Men’s Fitness & Men’s Physique and within Men’s Physique for Teen, Masters & Novice to Open for a second event. Additional entry fee of $30.00 for crossovers.

TANNING: On site/Backstage Tanning will be provided by Hot Spot Tanning of Sarasota. Tanning will be available to the competitors on Thursday at the host hotel (Gallery One-Ft.Lauderdale Doubletree Suites Hotel)and Friday & Saturday backstage at the War Memorial Auditorium. (NPC), Peter W. Potter, USA Sports Promotions of Florida, Inc., Manuel Mair, One to One Fitness, War Memorial Auditorium, the sponsors and them or their agents, representatives and assignees for any and all injuries suffered by me at said NPC Southern States Men’s Fitness & Men’s Physique Championships and in travel to and from said contest. In consideration of being allowed to participate in any way in the 2013 MANUEL MAIR NPC SOUTHERN STATES MEN’S FITNESS & PHYSIQUE (OPEN, TEEN & MASTERS) CHAMPIONSHIPS promoted by USA Sports Promotions of FL, Inc. and Manuel Mair and sanctioned by the National Physique Committee of the USA, Inc. (“NPC”), its related events and activities, I, Please print name_____________________________________________________), the undersigned, acknowledge, appreciate and agree that: The risk of injury from the activities involved in this Event is significant, including the potential for permanent paralysis and death, and while particular skills, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and, I KNOWLINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE PROMOTER AND/OR THE NPC or others, and assume full responsibility for my participation; and, I willingly agree to comply with the stated and customary terms and conditions for participation, if, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the Promoter immediately; and,I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE, INDEMNIFY, AND HOLD HARMLESS THE PROMOTER AND THE NPC and each of their officers, officials, agents and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used for the activity (“Releasees”), WITH RESPECT TO ANY AND ALL INJURY DISABILITY, DEATH, or loss or damage to person or property associated with my presence at or participation in or in traveling to and from the Event, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

The undersigned further consents and agrees that the undersigned will comply with all rules and regulations of the Gallery One - Ft. Lauderdale Doubletree Suites Hotel including any rules or regulations prohibiting cooking or food preparation in the hotel rooms. The undersigned further agrees that the undersigned will indemnify and hold harmless the NPC and the Promoter from and against any damages caused by the undersigned to the Hotel or damages to a guest room or rooms occupied or registered to the undersigned. Finally, the undersigned acknowledges that the undersigned’s failure to comply with any rule or regulation of the Hotel or damage to any guest room occupied by or registered to the undersigned may result in disciplinary action against the undersigned including, but not limited to, suspension as a member of the NPC and/or competing in contests sanctioned by the NPC. And in further consideration of permission being granted to me to participate in the Event and its related events. I hereby grant the NPC, Promoter and/or any other NPC-approved video or entertainment organization and all of their agents, successors, licensees and assigns, the right to photograph or otherwise reproduce (whether by film, tape, still photography or otherwise) my voice, appearance and name, and to exhibit, distribute, transmit, and/or otherwise exploit any and all media, including without limitation, by means of still photography, motion pictures, radio, television, television motion pictures, video, printing on digital or any other medium now known or hereafter devised, including with respect also to any merchandising, advertising and/or publicity, and the right to use my name and information about me in any connection with any of the foregoing. The rights granted by me hereunder are granted for the entire universe and shall endure in perpetuity and no further compensation shall be payable to me at any time in connection therewith. Nothing contained herein shall be deemed to obligate the NPC, Promoter, and/or any other NPC-approved video or entertainment organization, to photograph or otherwise reproduce my voice, appearance or name, or to make use of any rights granted herein. I also understand that the aforementioned rights may be reassigned at any time without further consent. I understand that the NPC, the Promoter and/or any NPC-approved video or entertainment organization, are videotaping and photographing the Event in express reliance upon the foregoing, and I represent and agree that I am free to grant the rights granted to the NPC, Promoter and/or any other NPC-approved video or entertainment organization hereunder. I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVE UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

FOR PARENTS/GUARDIANS OF PARTICIPANTS OF MINORITY AGE (UNDER AGE 18 AT TIME OF REGISTRATION) This is to certify that I, as parent/guardian with legal responsibility for this participant, do consent and agree to his/her release as provided above of all the Releasees, and, for myself, my child and our heirs, assigns, and next of kin, I release and agree to indemnify and hold harmless the Releasees from any and all liabilities incident to my minor child’s involvement or participation in these programs as provided above, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES, to the fullest extent permitted by law.
